Register for online proxy access to medical records (children under 12 and carers)

Use this service to request proxy access for a child under 12 years or a carer.

You can use this service if you:

• are registered at the surgery

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of

Once you have completed this form please either upload or visit the practice with two forms of identification. One of these items should include your photograph.

We will then issue you a username and password.
